A healthcare organization was dealing with overloaded phone lines, driven mostly by appointment requests. Voxivra designed a conversational appointment voicebot connected to a real-time calendar, able to prioritize urgent calls and send patients automatic reminders.
Context
As in many healthcare settings, administrative staff answered the phone while also greeting patients in person. Most calls were about booking, changing or cancelling appointments. At peak times, patients waited a long time or called back repeatedly, and no-shows left slots unused.
Challenge
- Absorb call volume without adding administrative staff.
- Never delay an urgent call behind routine requests.
- Keep the calendar reliable: no double bookings, availability always current.
- Reduce no-shows to recover appointment slots.
- Protect health information shared during calls.
- Stay accessible to every patient, including those uncomfortable with automated systems.
Solution
The approach combined a conversational voicebot, tight calendar integration and prioritization rules defined with the clinical team.
Natural language dialogue
Patients state their request freely: “I’d like to move my Thursday appointment.” Speech-to-text (STT), a language understanding engine and neural text-to-speech (TTS) make the exchange natural. Keypad input remains available as an alternative for patients who prefer it.
Real-time calendar
The voicebot checks availability at the time of the call, offers slots that match the reason for the visit and the practitioner, then writes the appointment directly into the system. Safeguards prevent conflicts when several callers target the same slot.
Smart prioritization
From the start of the call, the voicebot looks for signs of a priority situation based on rules set with clinical staff. These calls are transferred to a person immediately, and the appropriate safety instructions are given. The voicebot makes no diagnosis.
Automatic reminders
Before each appointment, an outbound reminder lets patients confirm, cancel or reschedule. Freed slots become available again right away. This builds on our call automation expertise.
Data protection
Data collection limited to what is needed, encrypted communications, defined retention periods for recordings and transcripts, and hosting choices aligned with data residency requirements.
Integration with existing telephony
The voicebot was placed in front of the existing phone system, so transfers reach the usual extensions and queues. Staff kept their tools and simply received fewer, better-qualified calls, with the patient’s request already identified.
Results
- 80% of calls automated end to end.
- 45-second response time.
- 40% more capacity to handle requests.
- Emergency prioritization built into the flow.
- Automatic reminders before every appointment.
- 30% fewer no-shows.
Key takeaways
- Calendar integration is the heart of the project. Without real-time reads and writes, a voicebot is just taking messages.
- Clinical safety is defined with clinicians. Prioritization and transfer rules are not a technical matter but a shared decision.
- Reminders directly increase capacity. Every no-show avoided is a slot returned to another patient.
- People stay reachable. An easy transfer to staff keeps patients’ trust, especially older patients.
